The MD.2 provides a secure online order form for you to place your order.
Ordering Online only takes a few moments. To place an order online we will need the following information about the person who will be USING the MD.2:
- Name
- Address
- Telephone number
- Date of Birth
- Names and telephone numbers of at least two Personal Contacts who live or work within 15 minutes of the person who will be USING the system, and who can come and check on them in person.
- Payment information. You can pay online either by credit card or by check.
Please note: to activate your MD.2, you will need to complete the following 2 forms for installation. You may click on each form, fill in the information as accurately as possible and submit the form by clicking the "Submit" button when you are finished.
